Digital ID cards can be accessed 24/7 by creating an account with UHCSR online at uhcsr.com/myaccount or by downloading the UHCSR mobile app to your smart phone or device. To create your account, you will need to use your student ID number.

All degree-seeking domestic undergraduate students enrolled at University of North Texas (Denton) are eligible to enroll in this insurance plan on a voluntary basis, provided the following criteria are met:

Students must be enrolled in a minimum number of credit hours (undergraduate students - six (6) hours; graduate students - three (3) hours) and no more than 50% of the total credit hours taken can be delivered in an internet/online instruction format.

Credit hours taken during a three-week course offering will not count towards meeting the above eligibility criteria.

Eligible Dependents of SHIP plan participants may be enrolled on a voluntary basis.

All registered International students, scholars or other persons with a current passport and student visa engaged in educational activities at UNT Denton are eligible and must be enrolled in the insurance plan on a hard waiver basis. This includes International students enrolled in Intensive English Language Institute (IELI), Curricular Practical Training, Internship, Practicum, Study Abroad or other credit or non-credit activity as a bona fide UNT student. International students participating in Optional Practical Training (OPT) or Academic Training (AT) are eligible to enroll on a voluntary basis for up to a year (unless approved for one additional year). Students must have been enrolled in SHIP as a student. If OPT/AT students have a gap in SHIP coverage while in the OPT/AT program, they must provide proof of their loss of coverage to re-enroll in SHIP.

Eligible Dependents of SHIP plan participants may be enrolled on a voluntary basis.

Domestic PhD Students

All Domestic PhD students are required to enroll in University of North Texas Student Health Insurance Plan (SHIP) unless a waiver is submitted and approved. The cost of the insurance coverage is not included in your tuition or fees and will appear as an additional charge on your account.  Students can request a waiver to SHIP but must demonstrate that they have equivalent, alternative health insurance coverage. In order to be approved for a waiver, your alternative health coverage must meet or exceed the requirements as set forth below and be submitted prior to the waiver deadline.  Notification of acceptance or rejection of this request will be sent to your school’s email within seven business days. 

1. The alternative coverage must meet the following minimum requirements: 

1. Provide the Essential Minimum Benefits required by the PPACA with no annual limits. A list of the Essential Minimum Benefits can be found here: healthcare.gov/glossary/essential-health-benefits/ . 

1. Contain no exclusions for pre-existing conditions. 

1. Covers 100% of Preventive Care as defined by the PPACA. A list of these preventive services can be found here: healthcare.gov/coverage/preventive-care-benefits/. 

1. Policies annual Deductible of no more than $2,000 

1. If your alternate coverage meets the above minimum requirements, submit electronic copies of the following documents with your online waiver request: 

• A scanned copy of the front and back of your health insurance ID card indicating the student as a covered member. 

NOTE: Healthcare Sharing plans will NOT be accepted by the University for waiver approval.

Continuation coverage is available to students who have graduated or are no longer eligible for Student Health Insurance. Students and their dependent(s) can purchase three (3) months of additional medical coverage. Students must have been covered for at least three (3) continuous months before coverage terminated. Application for Continuation of Coverage must be made and premium must be paid directly to Academic HealthPlans within 14 days after the expiration date of the Insured’s Student Health Insurance coverage.

All international students holding a visa are required to enroll in University of North Texas Student Health Insurance Plan (SHIP) unless a waiver is submitted and approved. The cost of the insurance coverage is not included in your tuition or fees and will appear as an additional charge on your account.  International students can request a waiver to SHIP but must demonstrate that they have equivalent, alternative health insurance coverage. In order to be approved for a waiver, your alternative health coverage must meet or exceed the requirements as set forth below and be submitted prior to the waiver deadline.  Notification of acceptance or rejection of this request will be sent to your school’s email within seven business days.

   1. Student is enrolled in a U.S. employer-provided group health plan that is PPACA compliant OR student is sponsored by a foreign government recognized by the United States or certain international, government-sponsored or non-governmental organizations, and covered under a health plan that is compliant with the Affordable Care Act (ACA).
   2. If the health plans above do not include medical evacuation and repatriation, a rider must be purchased providing coverage at equal limits to SHIP.
2. The alternative coverage must meet the following minimum requirements:

All Exchange students holding a visa are required to enroll in University of North Texas Student Health Insurance Plan (SHIP) unless a waiver is submitted and approved. The cost of the insurance coverage is not included in your tuition or fees and will appear as an additional charge on your account. Exchange students can request a waiver to SHIP but must demonstrate that they have equivalent, alternative health insurance coverage. In order to be approved for a waiver, your alternative health coverage must meet or exceed the requirements as set forth below and be submitted prior to the waiver deadline.  Notification of acceptance or rejection of this request will be sent to your UNT email within seven business days.

1. Criteria to submit a waiver request:
   1. Student is sponsored by a foreign government recognized by the U.S. or certain international, government-sponsored or non-governmental organizations OR
   2. Student is enrolled in an U.S. employer-provided group health plan OR
   3. Student policy is issued through the student’s home government or student’s home university where concurrently enrolled.
2. The alternative coverage must meet the following minimum requirements:
